The GENIE test is a specialized blood test that looks at which genes in your immune cells are turned up or turned down. Unlike a genetic test that looks for inherited mutations, GENIE is looking primarily at gene activity, that is how your immune system is responding at the time the blood is drawn.
You can think of your DNA as a large instruction manual. Most of those instructions remain the same throughout your life, but your cells continually decide which instructions they need to use. GENIE looks at patterns of this gene expression.
What Are We Looking For?
GENIE is primarily used in patients being evaluated for chronic inflammatory response syndrome (CIRS) and related illnesses.
The test looks for patterns of gene activity that may suggest the immune system is responding to particular types of environmental or biological stress. These may include patterns associated with:
- Water-damaged buildings and mold-related exposure
- Actinobacteria
- Endotoxins from certain bacteria
- Other inflammatory or immune-system responses
GENIE may also provide information about biological pathways involving inflammation, immune regulation, cellular metabolism, coagulation, and other physiologic processes.
For example, if GENIE shows a pattern interpreted as being associated with Actinobacteria, it does not mean that Actinobacteria were found in your bloodstream or that you have an Actinobacteria infection. Rather, the pattern of gene activity is interpreted as being consistent with an immune response that has been associated with that type of exposure.
How Is This Different From a Mold Test?
GENIE does not directly measure mold, mycotoxins, Actinobacteria, or endotoxin in your body or home.
Environmental tests such as ERMI, HERTSMI-2, Actinobacteria testing, or endotoxin testing examine samples from the environment.
GENIE examines your biological response.
One way to think about the difference is, environmental testing asks, “What might be present in the building?”
GENIE asks, “What is your immune system doing?”
When the two types of information agree, they can sometimes provide useful clues about what environmental factors deserve further investigation.
Is GENIE a Diagnostic Test?
Not by itself.
GENIE is a relatively specialized test, and the interpretation of its gene-expression patterns, particularly their attribution to specific environmental exposures, is based largely on the CIRS/Shoemaker diagnostic and treatment framework. These applications have not been established to the same degree as routine laboratory tests used for conditions such as diabetes, thyroid disease, or anemia.
Therefore, a GENIE result should be interpreted alongside your symptoms, medical history, physical examination, conventional laboratory testing, and environmental assessment.
The goal isn’t to treat a laboratory report. The goal is to use the information as another piece of evidence in understanding why the immune system may remain activated and whether there is an ongoing exposure that needs to be identified and addressed.
