Integrative Supplements for Chemotherapy-Induced Peripheral Neuropathy (CIPN)
Some chemotherapy drugs, especially oxaliplatin (FOLFOX/CAPOX), paclitaxel, docetaxel, vincristine, and bortezomib, may damage the nerves in the hands and feet, causing numbness, tingling, burning, pain, or balance problems. While no supplement has been definitively proven to prevent neuropathy, several have shown encouraging results in clinical studies.
Supplements with the Best Evidence
Omega-3 Fish Oil
Dose: 1,000–2,000 mg EPA + DHA daily
- May reduce inflammation and oxidative stress
- Supports healthy nerve cell membranes
Alpha-Lipoic Acid (ALA)
Dose: 300–600 mg daily
- Potent antioxidant that supports mitochondrial function and nerve repair
- Discuss timing with your oncologist, as some prefer starting after chemotherapy is completed
Melatonin
Dose: 3–10 mg at bedtime (occasionally higher under physician guidance) – we often dose 4x/day
- May protect nerves from oxidative injury
- Also improves sleep quality and may reduce fatigue
Vitamin D3
Maintain a blood level of approximately 40–60 ng/mL. Supplementation is most beneficial if you are deficient. Repeat blood levels every 3 months to insure stability
Vitamin B12
Supplement only if deficiency is present or suspected. Healthy B12 levels are essential for normal nerve function.
Supplements with Emerging Evidence
N-Acetylcysteine (NAC) – 600 mg once or twice daily
Supports the body’s natural production of glutathione, an important antioxidant.
Glutamine – 10 g two to three times daily
May lessen neuropathy in some patients receiving oxaliplatin. Discuss with your oncologist before use.
Magnesium (preferably glycinate) – 200–400 mg elemental daily
Supports normal nerve and muscle function.
Supplements Requiring Caution
Acetyl-L-Carnitine (ALC) was once considered promising, but newer studies suggest it may worsen neuropathy during some chemotherapy regimens. Routine use during active chemotherapy is generally not recommended.
Additional Therapies Under Investigation
Curcumin • Palmitoylethanolamide (PEA) • Benfotiamine • Lion’s Mane mushroom • Intravenous glutathione • Plasmalogens
These remain promising but require additional research before routine recommendation.

The best results come from combining supplements with healthy lifestyle habits:
- Eat a Mediterranean-style, protein-rich diet.
- Exercise regularly as tolerated.
- Sleep well and stay hydrated.
- Limit alcohol.
